When it comes to the Avowed A Cure For Rage Quest choice, you should help Grakohr find a cure. There are long-term benefits to making him a friend that reveal themselves later in the game after you've left Galawain's Tusks and finished exploring The Garden.
In the Side Quest "A Cure for Rage," you'll come across an Ogre named Grakohr holding the Pagrun woman Gilyn hostage. Grakohr was attacked by a merchant caravan (supposedly unprovoked), and Gilyn is the sole survivor — and his hostage. Grakohr is suffering from a disease known as Molder Lung, and he demands your help.
Should you decide to kill Grakohr, you'll get the Unique Two-Handed Melee Great Axe "Limb-Taker." Its Power Attacks deal some Bleed Accumulation, and it also deals +10% bonus Bleeding Damage on hit. This reward, however, pales in comparison to what you'll get if you decide to help him.
You'll have to run all around Galawain's Tusks to collect the needed ingredients, but it can be worth it. Curing Grakohr nets you a Unique Item, yes — but it also pays off in the game's final battle.

First and foremost, helping Grakohr will have two immediate benefits: you'll get access to another merchant when Gilyn sets up in the Southern Warden Tower and Grakohr will give you the Unique Amulet "Stoneheart." This Amulet gives you the ability to summon a Ruinous Blight to fight with you.
After you've decided Sapadal's Fate and received your final Godlike Ability, you'll have to participate in the Siege of Paradis. Grakohr will be there to lend his assistance, helping you in one of the battles against the Steel Garrote soldiers invading the town. He then says that he's pledging his life to protecting people thanks to your kindness.
Ultimately, it comes down to what you think is more important — do you want a cool two-handed axe, or would you prefer an amulet and formidable ally in the game's toughest battle? Every little bit of help counts in the end, and that's why you should probably help cure Grakohr.
